Latar Belakang

Angka harapan hidup di Indonesia meningkat dengan layanan kesehatan yang lebih baik. Namun hal ini menyebabkan peningkatan jumlah pasien dengan penyakit tidak menular yang bersifat kronis dan tidak dapat disembuhkan seperti kanker, gagal organ dan penyakit degenerative. Prevalensi penyakit kanker misalnya, telah meningkat di Indonesia, dari 1.4% o di tahun 2012 menjadi 1.79% o in 2018. Permasalhan penyakit kanker di Indonesia makin bertambah, karena sebagian besar ditemukan dalam stadium lanjut. Data dari RS Kanker Dharmais menunjukkan 63% kanker berada dalam stadium paliatif, yang memerlukan pengobatan yang bertujuan untuk memperpanjang harapan hidup, bukan menyembuhkan. Sebagian besar pasien kanker akan masuk dalam stadium terminal, yaitu suatu periode progresifitas penyakit yang tidak dapat dihentikan dengan modalitas apapun, dan membawa kepada kematian. Di Indonesia kanker merupakan penyebab kematian ke-4 setelah stroke, hipertensi, dan diabetes (DepKes, 2011)

Seperti di negara- negara dengan pendapatan rendah dan menegah lainnya, penyakit tidak menular menimbulkan beban bagi negara. Di Indonesia, penyakit kanker memberikan beban pembiayaan yang sangat besar bagi pemerintah dan masyarakat. Data Badan Pengelola Jaminan Sosial (BPJS) tahun 2014 menunjukkan pengeluaran dana untuk pasien kanker sebesar 2,5 trilliun rupiah dan merupakan terbanyak ke-3 setelah penyakit jantung dan gagal ginjal (Biro Komunikasi dan Pelayanan Masyarakat Sekretaris Jendral Kementrian Kesehatan RI, 2016).

Penyakit kronis, menimbulkan gejala fisik, gangguan psikologis, kesulitan sosial, dan masalah spiritual yang berat selama menjalani pengobatan sehingga memberikan dampak negatif terhadap kualitas hidup, biaya pengobatan dan bahkan harapan hidup (Zhi & Smith, 2015). Pasien memerlukan tatalaksana yang tidak bersifat disease-centered, namun yang berdasar prinsip patient’s-centered (Lamont, 2005; Davis, et al., 2015; Rietjen, 2016) dan layanan yang bersifat patient and family-centered care (Kirk & Mahon, 2010).

Tata laksana pasien stadium terminal adalah salah satu indikator kualitas layanan pasien pdengan penyakit kronis seperti kanker (Grendarova, 2015). Namun stadium terminal sering tidak terdiagnosis, sehingga tidak mendapat tatalaksana sebagaimana mestinya. Sebaliknya, mendapatkan intervensi yang dirancang untuk mencapai kesembuhan. Tindakan medis standar atau pengobatan agresif, pada pasien stadium terminal bisa bersifat iatrogenic dan akan menjadi sia-sia dan tidak efisien (Bailey, 2005). Tata laksana stadium terminal yang adekuat berfokus pada kualitas sisa hidup, kenyamanan, dan penghindaran tindakan sia-sia atau intervensi invasif yang menghalangi kematian yang tenang, damai, dan bermartabat (Kennedy, 2014).

Apa yang dimaksud dengan Perawatan Paliatif?

Perawatan paliatif adalah suatu pendekatan yang ditujukan bagi pasien dnegan penyakit yang dpat mengancam jiwa yang bertujuan untuk memperbaiki kualitas hidup pasien dan keluarganya yang mengalami masalah yang berhubungan dengan penyakit yang dapat mengancam jiwa melalui

pencegahan dan mengurangi penderitaan melalui deteksi dini dan asesmen yang seksama serta pengobatan nyeri dan masalah lain baik fisik, psikologis, dan spiritual (WHO, 2002). Perawatan paliatif dilakukan oleh tim multidisiplin yang bekerja secara interdisiplin untuk mencapai tujuan tatalaksana yang ditentukan bersama (Doyle, et al; 1999).

Integrasi perawatan paliatif pada penyakit yang dapat mengancam jiwa misalnya di bidang onkologi memberikan banyak manfaat bukan hanya bagi pasien tetapi bagi keluarga dan layanan kesehatan, termasuk dalam hal pembiayaan (Morisson, et al., 2011; Putranto, et al., 2017). Namun di Indonesia sampai saat ini belum berjalan dengan baik. Pada tahun 2018, Program Improving Access to Cancer Clinical Trial (ImPACT) melaporkan bahwa dari pasien yang memerlukan perawatan paliatif di Indonesia, baru 1% yang mendapatkan layanan (IARC, 2018).

Pengembangan layanan kesehatan yang komprehensif memerlukan tidak hanya pengobatan yang bersigfat kuratif tetapi juga perawatan paliatif. Perawatan paliatif di Indonesia ditujukan terutama bagi pasien stadium terminal, yang diatur melalui Surat Keputusan Menteri Kesehatan RI Nomor 812/Menkes/SK/VII/2007. SK tersebut menyebutkan bahwa dalam rangka meningkatkan kualitas pelayanan kesehatan bagi pasien dengan penyakit yang belum dapat disembuhkan, selain dengan perawatan kuratif dan rehabilitatif juga diperlukan perawatan paliatif.

Disamping mendapatkan tatalaksana konvensional untuk mencapai tujuan pengobatan, pasien dengan penyakit yang memiliki harapan hidup terbatas harus mendapatkan kesempatan memperoleh perawatan paliatif yang berkualitas sepanjang perjalanan penyakit untuk meningkatkan kualitas hidup dan mengurangi beban keluarga serta mencapai efisiensi layanan kesehatan. Pengembangan perawatan paliatif memerlukan kebijakan pemerintah, tenaga kesehatan yang memiliki pengetahuan dan ketrampilan tentang perawatan paliatif, masyarakat yang sadar akan kebutuhan dan terlibat dalam perawatan paliatif dan ketersediaan obat-obat yang diperlukan.

Pendahuluan

Dalam mengatasi pandemi COVID-19 Indonesia menghadapi sejumlah tantangan utama, mulai akses terhadap layanan kesehatan yang masih belum merata hingga rasio fasilitas kesehatan dan jumlah ketersedian tenaga kesehatan serta persebarannya yang tidak sebanding dengan jumlah penduduk Indonesia. Menghadapi tantangan tersebut, teknologi informasi mempunyai peran yang sangat penting sebagai salah satu solusi mengatasi pandemi COVID-19. Adanya pandemi serta semakin berkembangnya teknologi mendorong Kemenkes RI untuk segera melakukan transformasi digital kesehatan sebagai lompatan menuju sektor kesehatan Indonesia yang semakin maju, berkembang, dan berkeadilan.

Pada akhir tahun 2021, Kementerian Kesehatan mempublikasikan Cetak Biru Transformasi Digital Kesehatan. Peluncuran Cetak Biru sebagai Strategi Transformasi Digital Kesehatan 2024 ini diharapkan dapat memberikan gambaran kepada seluruh pelaku industri di bidang kesehatan terkait arah dan peta jalan transformasi digital kesehatan di Indonesia dalam beberapa tahun mendatang. Peluncuran Cetak Biru sebagai Strategi Transformasi Digital Kesehatan sebagai strategi jangka panjang untuk menanggulangi COVID-19. Strategi transformasi digital tersebut berfokus pada ekosistem kesehatan, efisiensi layanan, dan integrasi data untuk kebijakan berbasis data.

Berkenaan dengan hal tersebut, dukungan pada pengembangan data dan aplikasi kesehatan perlu diiringi dengan semangat untuk fokus bersama dalam upaya mewujudkan Indonesia sehat secara kolaboratif, meningkatkan sistem pelayanan kesehatan yang terintegrasi dan berkelanjutan mulai dari sektor kesehatan dalam pelayanan sejak awal kehidupan di dalam kandungan hingga pelayanan kesehatan terpadu bagi pasien lansia.

Kementerian kesehatan juga mendukung upaya digitalisasi di tingkat fasilitas kesehatan, dibuktikan dengan dikembangkan berbagai inovasi sistem kesehatan salah satunya dengan adanya rekam kesehatan elektronik. Rekam kesehatan elektronik memiliki peran penting pada beberapa aspek dan masih terus dikembangkan, salah satunya agar dapat terintegrasi dengan berbagai fasilitas kesehatan untuk mendukung satu data kesehatan. Dalam pengembangan sistem rekam kesehatan elektronik ada tantangan yang perlu diperhatikan seperti dalam hal proteksi dan keamanan data. Dalam hal ini diperlukan sistem keamanan dan proteksi canggih, kolaborasi serta inovasi dari sudut pandang berbagai pihak dalam mewujudkan pengembangan sistem rekam kesehatan elektronik agar sesuai dengan kebutuhan pengguna dalam mendukung peningkatan kualitas pelayanan kesehatan kepada pasien.

Selain rekam kesehatan elektronik, terdapat istilah sistem pencatatan data kesehatan individu pasien berupa rekam medis elektronik dan personal health records. Perbedaan dari ketiga jenis sistem pencatatan ini dari segi penggunaan, rekam kesehatan elektronik fokus pada informasi kesehatan pasien secara keseluruhan, melebihi data klinis standar yang dikumpulkan di fasilitas pelayanan kesehatan. Sistem ini dibangun untuk membagi informasi dengan penyedia layanan kesehatan lainnya. Sedangkan rekam medis elektronik pada dasarnya sama dengan rekam kesehatan elektronik, namun rekam medis elektronik lebih spesifik, pencatatan medis pasien digital ini digunakan untuk satu pelayanan kesehatan tunggal. Kemudian untuk personal health records merupakan rekam kesehatan elektronik yang dapat diakses oleh pasien atau keluarga pasien yang dapat berupa web atau aplikasi berbasis mobile. Konsep personal health records akan memungkinkan pasien atau keluarga pasien memiliki aksesibilitas terhadap data kesehatannya (patient centeredness and clinical decision making).

Tujuan

Seminar ini bertujuan untuk mendiskusikan Rekam Kesehatan Elektronik dari perspektif Kementerian Kesehatan, pengembang dan pemerintah daerah.

Latar belakang

Praktik keperawatan berbasis bukti sangat penting dalam mendukung perawatan berkualitas tinggi untuk mengoptimalkan manfaat dan hasil terbaik bagi pasien. Penelitian menunjukkan hasil yang lebih baik ketika evidence-based nursing terbaik diterapkan dalam pemberian perawatan pasien. Namun, menerapkan evidence based dalam praktik keperawatan dilapangan merupakan proses yang panjang dan penuh tantangan. Meskipun telah banyak literatur tentang strategi yang efektif dalam menerapkan, banyak profesional kesehatan masih belum familiar dan kesulitan dalam praktek penerapannya.

Beberapa tahun terakhir kesadaran akan pentingnya berlatih, menerapkan dan mempertahankan praktik berbasis bukti dalam lingkungan praktik menjadi tantangan tersendiri. Terdapat berbagai kemungkinan alasan keterbatasan dalam penerapan praktik berbasis bukti saat ini. Profesional kesehatan mungkin belum familiar dan kurang memiliki keterampilan dalam mencari dan mengevaluasi temuan penelitian, beban kerja disebut sebagai penghalang untuk mengakses database yang diperlukan, dan fasilitas pendukung yang masih perlu ditingkatkan. Untuk menjawab tantangan ini, diperlukan kerjasama berbagai pihak mulai dari pemangku kebijakan sampai perawatan diberbagai lini pelayanan keperawatan.

Membuat perubahan berarti kerja ekstra yang cukup besar bagi para praktisi garis depan. Untuk itu, Alumni PSIK FKKMK UGM Angkatan 2004 bekerjasama dengan DPK PPNI Stikes Notokusumo DI Yogyakarta, akan mengadakan seminar sehari untuk mengoptimalkan penerapan praktek berbasis bukti dalam pelayanan keperawatan. Seminar ini mengangkat tema putting evidence into nursing practice.

Tujuan Kegiatan

Adapun tujuan kegiatan ini adalah sebagai berikut

• meningkatkan pemahaman, kesadaran, dan komitmen perawat dalam menerapkan praktek berbasis bukti dalam praktek keperawatan
• wadah diskusi dan sharing ide untuk meningkatkan pelayanan keperawatan di berbagai
• sebagai sarana silahturahim dalam mempererat hubungan alumni dan seluruh civitas PSIK FKKMK UGM

Latar Belakang

Kunjungan ke fasilitas pelayanan kesehatan dapat membuat orang gugup karena berbagai alasan. Beberapa yang dirasakan oleh kelompok rentan seperti orang dengan HIV dan populasi kunci adalah merasa tidak nyaman mengungkapkan informasi sensitif pada petugas kesehatan. merasa takut informasi terkait status kesehatannya akan menyebar ke lingkungan sekitar setelah kunjungan ke fasilitas pelayanan kesehatan dan mendapat stigma dan diskriminatif saat di layanan kesehatan. Pada Orang dengan disabilitas, fasilitas pelayanan kesehatan belum semuanya menyelenggarakan layanan yang ramah bagi difabel sebagai contoh: belum semua Puskesmas dan Rumah Sakit di Indonesia dilengkapi dengan jalur khusus difabel, dan tidak ada juru bahasa isyarat yang ditempatkan di layanan kesehatan.

Menciptakan layanan dan lingkungan fasilitas kesehatan yang dipercaya dan nyaman bagi pasien adalah salah satu tujuan penting dalam penyelenggaraan pelayanan kesehatan. Oleh karena mendapat layanan kesehatan secara layak dan sama adalah hak semua orang, maka petugas kesehatan harus siap melayani pasien dari semua suku, ras, agama, usia, ragam kondisi fisik dan latar belakang sosial. Ketika pasien memiliki pengalaman buruk seperti mendapat stigma dan diskriminasi dari petugas hanya karena pasien berpenampilan berbeda, memiliki orientasi seksual berbeda, atau memiliki disabilitas fisik atau mental, besar kemungkinan pasien akan menyembunyikan informasi penting tentang kesehatannya. Yang lebih buruk lagi adalah mereka tidak akan datang kembali ke fasilitas kesehatan hingga putus pengobatan.

Praktek inklusif dalam pelayanan kesehatan adalah layanan yang mengacu pada akses, interaksi pasien-petugas dan perlakuan yang adil bagi pasien dalam mengakses layanan kesehatan. Profesionalitas petugas kesehatan dalam memberikan layanan tidak hanya berlaku pada pasien umum saja tetapi juga pada pasien dari populasi yang rentan mengalami stigmatisasi seperti orang dengan disabilitas, orang dengan HIV, transgender, waria, WPS, penasun, LSL, dan lain-lain. Mereka juga termasuk dalam kelompok yang memiliki kerentanan lebih tinggi di masa Pandemi COVID-19 terkait penyakit penyerta (seperti HIV, TB), masalah ekonomi, hingga diskriminasi dari lingkungan sosialnya.

Mengingat puskesmas merupakan “pintu depan” sistem kesehatan yang memberikan landasan bagi penguatan kesehatan di masyarakat, maka penguatan layanan inklusif bagi semua pasien di masa pandemi COVID-19 sangat diperlukan. Seminar ini dapat meningkatkan literasi, mengangkat contoh pengalaman di lapangan mengenai praktik-praktik pelayanan kesehatan yang inklusif terutama dalam konteks Pandemi COVID-19.

Tujuan

1. Menyajikan pengalaman dan pembelajaran (lesson learned) secara langsung dari lapangan mengenai upaya puskesmas dalam menyediakan layanan inklusif bagi kelompok rentan (misal pasien TB, HIV, Orang dengan disabilitas, dll) di masa Pandemi COVID-19
2. Memperkuat strategi dalam upaya mengembangkan layanan kesehatan yang inklusif

Latar Belakang

Angka harapan hidup di Indonesia meningkat dengan layanan kesehatan yang lebih baik. Namun hal ini menyebabkan peningkatan jumlah pasien dengan penyakit tidak menular yang bersifat kronis dan tidak dapat disembuhkan seperti kanker, gagal organ dan penyakit degenerative. Prevalensi penyakit kanker misalnya, telah meningkat di Indonesia, dari 1.4% o di tahun 2012 menjadi 1.79% o in 2018. Permasalhan penyakit kanker di Indonesia makin bertambah, karena sebagian besar ditemukan dalam stadium lanjut. Data dari RS Kanker Dharmais menunjukkan 63% kanker berada dalam stadium paliatif, yang memerlukan pengobatan yang bertujuan untuk memperpanjang harapan hidup, bukan menyembuhkan. Sebagian besar pasien kanker akan masuk dalam stadium terminal, yaitu suatu periode progresifitas penyakit yang tidak dapat dihentikan dengan modalitas apapun, dan membawa kepada kematian. Di Indonesia kanker merupakan penyebab kematian ke-4 setelah stroke, hipertensi, dan diabetes (DepKes, 2011)

Seperti di negara- negara dengan pendapatan rendah dan menegah lainnya, penyakit tidak menular menimbulkan beban bagi negara. Di Indonesia, penyakit kanker memberikan beban pembiayaan yang sangat besar bagi pemerintah dan masyarakat. Data Badan Pengelola Jaminan Sosial (BPJS) tahun 2014 menunjukkan pengeluaran dana untuk pasien kanker sebesar 2,5 trilliun rupiah dan merupakan terbanyak ke-3 setelah penyakit jantung dan gagal ginjal (Biro Komunikasi dan Pelayanan Masyarakat Sekretaris Jendral Kementrian Kesehatan RI, 2016).

Penyakit kronis, menimbulkan gejala fisik, gangguan psikologis, kesulitan sosial, dan masalah spiritual yang berat selama menjalani pengobatan sehingga memberikan dampak negatif terhadap kualitas hidup, biaya pengobatan dan bahkan harapan hidup (Zhi & Smith, 2015). Pasien memerlukan tatalaksana yang tidak bersifat disease-centered, namun yang berdasar prinsip patient’s-centered (Lamont, 2005; Davis, et al., 2015; Rietjen, 2016) dan layanan yang bersifat patient and family-centered care (Kirk & Mahon, 2010).

Tata laksana pasien stadium terminal adalah salah satu indikator kualitas layanan pasien pdengan penyakit kronis seperti kanker (Grendarova, 2015). Namun stadium terminal sering tidak terdiagnosis, sehingga tidak mendapat tatalaksana sebagaimana mestinya. Sebaliknya, mendapatkan intervensi yang dirancang untuk mencapai kesembuhan. Tindakan medis standar atau pengobatan agresif, pada pasien stadium terminal bisa bersifat iatrogenic dan akan menjadi sia-sia dan tidak efisien (Bailey, 2005). Tata laksana stadium terminal yang adekuat berfokus pada kualitas sisa hidup, kenyamanan, dan penghindaran tindakan sia-sia atau intervensi invasif yang menghalangi kematian yang tenang, damai, dan bermartabat (Kennedy, 2014).

Apa yang dimaksud dengan Perawatan Paliatif?

Perawatan paliatif adalah suatu pendekatan yang ditujukan bagi pasien dnegan penyakit yang dpat mengancam jiwa yang bertujuan untuk memperbaiki kualitas hidup pasien dan keluarganya yang mengalami masalah yang berhubungan dengan penyakit yang dapat mengancam jiwa melalui

pencegahan dan mengurangi penderitaan melalui deteksi dini dan asesmen yang seksama serta pengobatan nyeri dan masalah lain baik fisik, psikologis, dan spiritual (WHO, 2002). Perawatan paliatif dilakukan oleh tim multidisiplin yang bekerja secara interdisiplin untuk mencapai tujuan tatalaksana yang ditentukan bersama (Doyle, et al; 1999).

Integrasi perawatan paliatif pada penyakit yang dapat mengancam jiwa misalnya di bidang onkologi memberikan banyak manfaat bukan hanya bagi pasien tetapi bagi keluarga dan layanan kesehatan, termasuk dalam hal pembiayaan (Morisson, et al., 2011; Putranto, et al., 2017). Namun di Indonesia sampai saat ini belum berjalan dengan baik. Pada tahun 2018, Program Improving Access to Cancer Clinical Trial (ImPACT) melaporkan bahwa dari pasien yang memerlukan perawatan paliatif di Indonesia, baru 1% yang mendapatkan layanan (IARC, 2018).

Pengembangan layanan kesehatan yang komprehensif memerlukan tidak hanya pengobatan yang bersigfat kuratif tetapi juga perawatan paliatif. Perawatan paliatif di Indonesia ditujukan terutama bagi pasien stadium terminal, yang diatur melalui Surat Keputusan Menteri Kesehatan RI Nomor 812/Menkes/SK/VII/2007. SK tersebut menyebutkan bahwa dalam rangka meningkatkan kualitas pelayanan kesehatan bagi pasien dengan penyakit yang belum dapat disembuhkan, selain dengan perawatan kuratif dan rehabilitatif juga diperlukan perawatan paliatif.

Disamping mendapatkan tatalaksana konvensional untuk mencapai tujuan pengobatan, pasien dengan penyakit yang memiliki harapan hidup terbatas harus mendapatkan kesempatan memperoleh perawatan paliatif yang berkualitas sepanjang perjalanan penyakit untuk meningkatkan kualitas hidup dan mengurangi beban keluarga serta mencapai efisiensi layanan kesehatan. Pengembangan perawatan paliatif memerlukan kebijakan pemerintah, tenaga kesehatan yang memiliki pengetahuan dan ketrampilan tentang perawatan paliatif, masyarakat yang sadar akan kebutuhan dan terlibat dalam perawatan paliatif dan ketersediaan obat-obat yang diperlukan.

Dokter merupakan profesi yang dianggap mulia oleh masyarakat karena nilai luhur dalam perawatan dan pertolongan sesama. Namun, tidak dapat dihindari di era disrupsi 4.0 ini peran dokter dituntut untuk berkembang sesuai jamannya dan juga untuk mengakomodasi personal interest yang beragam di kalangan dokter sendiri.Untuk mendiskusikan perubahan – perubahan peran dokter ini, angkatan 96 Kagamadok menyelenggarakan webinar dengan topik “Peningkatan trias peran dokter terutama dalam bidang: agent of science, agent of development,dan agent of change”.

Pengetahuan ini akanlebih mengalir jika dipaparkan oleh ahli yang berpengalaman menjalani bidangnya masing-masing; dan Angkatan 1996 Kagamadok beruntung memiliki anggota dengan jalur profesi yang beragam sesuai dengan trias peran dokter di atas. Dengan mengikuti seminar ini peserta diharapkan mendapat tambahan informasi dan menginspirasi terutama bagi mahasiswa kedokteran dan kesehatan, serta dokter umum dan spesialis, dan tenaga Kesehatan lainnya dalam pilihan karir dan ekspresi dirinya.
